Project: Emergency Department at St. Francis Referral Hospital, Ifakara, Tanzania

Background

The St Francis Referral Hospital (SFRH) serves a population of about 550’000 people and offers inpatient care for 370 patients. The hospital engages almost 400 persons including 20 doctors and 80 medical officers. So far, no proper emergency department exists at the SFRH. In 2014 the construction of an emergency department was started with the support of national and international funds (among others the Direktion für Entwicklung und Zusammenarbeit (DEZA), and will be finished in 2016. Up to now, the directory of SFRH did not find a Tanzanian doctor trained in emergency medicine and capable of implementing an emergency medicine processes with organization, triage and training of staff. However, such a position is urgently required to fulfill the hospital’s role as a teaching and referral hospital at a tertiary level. Nevertheless, one doctor is actually trained at the Dar Es Salaam University Hospital, and was designated by the directory as a partner of our project.

Project Aims

1. Organisation of an Emergency Department

  • Implementation of the emergency severity index (ESI) for patient triage
  • Implementation of a clinical shift operation


2. Training

  • Training of health care workers (medical doctors, assistant medical officers, clinical medical officers and nurses) in EM together with the Tanzanian Training Centre for International Health (TTCIH) at Ifakara.
  • Training of local medical personnel to become instructors in EM.
  • Training in sonography and focussed echocardiography, training of local medical personnel to become instructors/teachers in sonography and focussed echocardiography.

Objectives

Primary Objective:

  • To evaluate the impact of the implementation of a patient triage system (ESI), clinical shift operation, and training of medical personnel on mortality, time to diagnosis, and length of hospital stay.


Secondary Objectives:

  • To determine patient’s characteristics and diseases in order to adapt the services to locally specific needs.
  • To evaluate the impact of structured emergency ultrasound exams on time to diagnosis.
  • To analyse the present management of emergency department patients and to determine their in- and outpatient needs.
