BACKUP Health Cameroon - Improving Access to Malaria Care and Services for Children Under Five: Pilot Project

BACKUP Health is a GIZ programme working for the German Federal Ministry for Economic Cooperation and Development (BMZ) and is cofounded by the Swiss Agency for Development and Cooperation (SDC) since 2013.

Since 2002, BACKUP has supported public and civil society partners with more than 600 measures in 90 countries to better implement programmes funded by the Global Fund to Fight AIDS, Tuberculosis and Malaria (GFATM). The project ensures that grants are used effectively by focusing on target groups and strengthening health systems. In this way, it stimulates the effectiveness and sustainability of the measures funded.

Cameroon, as a high-impact country in the GF portfolio, absorbed $164.4 million between 2018 and 2020. For the next period from 2021 to 2023, Cameroon has received a funding allocation of nearly $250 million, of which approximately $101 million will go to malaria treatment and control programs. These funds are expected to be absorbed through the end of 2023.

The National Malaria Control Program (PNLP) is working towards Universal Health Coverage (UHC) to consolidate care and treatment programmes that will result in free and subsidized care. For the moment, the costs of the different treatments and services vary according to the subcategory of the population considered and differences in costs have been observed in the different regions and between urban and rural areas. In this context, it is important to monitor the implementation of free care for children under five years of age and to know the real costs that other categories of patients have to pay for diagnostic care and treatment in the districts.

This project aims to contribute to a better transparency of payment for malaria care and services, through the design of a strategy to monitor treatments and costs administered through the use of the openIMIS programme in selected health districts in Cameroon on a pilot basis.

With the potential offered by the openIMIS software, it will be possible to have transparent information on the distribution of treatments and their costs in the different health centers and thus to manage their management once they are introduced into the national CSU.

The four objectives that summarize the need that this project must meet can be achieved through the following four groups of activities:

  1. Estimation of the costs of malaria treatment services and care: The aim is to propose three cost models for the management of all malaria care: "basic", "standard" and "comprehensive". The objective of this activity is to calculate the costs of malaria care coverage for all and selected subgroups of the population in Cameroon.
  2. Design of the pilot and the roadmap: this involves assessing the policy conditions and making recommendations for the management of malaria care for children under 5 years of age, including the development of a roadmap for the implementation of the pilot and the development of a procurement scheme for care.
  3. openIMIS: feasibility and cost estimation. The goal is to customize openIMIS to meet the needs of malaria care management.
  4. Implementation of the openIMIS pilot in order to record and monitor the malaria health services provided in order to have information on their frequency and costs and thus have a tool for monitoring the funding dedicated to this disease.

Swiss TPH has surrounded itself with experts for the realization of this mandate with Oeconomia-Expertise for the health economics issues and Y-Note for the IT aspects in Cameroon.
openIMIS is an open source tool that is currently being used successfully in the free HIV care and treatment campaign in Cameroon as well as in the BEPHA community health insurance network and for which a pilot is being prepared for the Chèque Santé, also in Cameroon. The project will therefore be oriented according to the good practices observed in Cameroon. In addition, the project will be piloted in close collaboration with the National Malaria Control Program (NMCP) and the BACKUP Health team at GIZ.

The following four groups of activities will be implemented successively:

  1. Estimation of the costs of malaria treatment services and care
  2. Design of the pilot and roadmap
  3. openIMIS: feasibility and cost estimation
  4. Implementation of the openIMIS pilot

At the end of the project it is expected that the openIMIS pilot will be functional and that the possibilities of its scaling up will be known.

The project will pay particular attention to the specific target of children under 5 years of age, beneficiaries of the national policy of free malaria care, as well as pregnant women. The different components of the project (cost study, roadmap, data monitoring with openIMIS) will therefore specifically distinguish these targets from the rest of the population.

The following direct implementation partners are noted:

  • The steering partners, notably GIZ BACKUP/P4H, the National Malaria Control Program (PNLP) of the Ministry of Health (MINSANTE) at the central level.
  • The implementation partners identified at this stage: the IT unit of MINSANTE; the regional health authorities, FRPS and health facilities (FOSA) involved in the activities, particularly for the cost study and for the openIMIS pilot.

BACKUP Health Cameroon - Amélioration de l’accès aux soins et services lies au paludisme pour les enfants de moins de 5 ans au Cameroun : Projet pilote

BACKUP Santé est un programme de la GIZ qui travaille pour le ministère fédéral allemand de la Coopération économique et du Développement (BMZ) et bénéficie d'un cofinancement de L'Agence Suisse du développement et de la coopération (DDC) depuis 2013.

Depuis 2002, BACKUP a soutenu des partenaires publics et de la société civile avec plus de 600 mesures dans 90 pays, afin de mieux mettre en œuvre les programmes financés par le Fonds mondial de lutte contre le Sida, la Tuberculose et le Paludisme (FM). Le projet permet de s'assurer que les subventions sont utilisées de manière efficace en se concentrant sur les groupes cibles et en renforçant les systèmes de santé. De cette manière, il stimule l'efficacité et la durabilité des mesures financées.

Le Cameroun, en tant que pays à fort impact dans le portefeuille du FM, a absorbé 164,4 millions de dollars entre 2018 et 2020. Pour la prochaine période allant de 2021 à 2023, le Cameroun a reçu une allocation de financement de près de 250 millions de dollars, dont environ 101 millions de dollars iront au traitement et aux programmes de lutte contre le paludisme. Ces fonds devraient être absorbés jusqu'à la fin de l'année 2023.

Le programme national de lutte contre le Paludisme (PNLP) travaille dans la perspective de la Couverture Sanitaire Universelle (CSU), pour consolider des programmes de soins et traitements devant aboutir à une gratuité et des subventions.

Pour le moment, les coûts des différents traitements et services varient selon la sous-catégorie de la population considérée et des différences dans les coûts ont été observés dans les différentes régions et entre zones urbaines et rurales.

Dans ce contexte, il importe d’assurer un suivi de la mise en œuvre de la gratuité des soins pour les enfants de moins de cinq ans et de connaître les coûts réels que les autres catégories de patients doivent payer pour les soins de diagnostic et traitement dans les districts.

Ce projet vise à contribuer à une meilleure transparence de paiement des soins et services liés au paludisme, par la conception d’une stratégie de suivi des traitements et coûts administrés par l’usage du programme openIMIS dans certains districts de santé au Cameroun à titre pilote.

Avec le potentiel offert par le logiciel libre de droits openIMIS, il sera possible de disposer d’une information transparente sur la distribution des traitements et leurs coûts dans les différents centres de soins et ainsi permettre de gérer leur prise en charge une fois qu’ils seront introduits dans la CSU nationale.

Les quatre objectifs qui synthétisent le besoin auxquels ce projet doit répondre peuvent se réaliser à travers les quatre groupes d’activités suivantes :

  1. Estimation des coûts des services et soins liés au traitement du paludisme : La finalité est de proposer trois modèles de coûts pour la prise en charge de tous les soins de Paludisme : « de base », « standard » et « complet ». L’objectif de cette activité est de calculer les coûts de la couverture des soins de paludisme pour l’ensemble et certains sous-groupes de la population au Cameroun.
  2. Conception du pilote et de la feuille de route : il s’agit d’évaluer les conditions politiques et faire des recommandations pour la prise en charge des soins de paludisme d´enfant de moins de 5 ans, dont l’élaboration d’une feuille de route pour la mise en place du pilote et l’élaboration d’un schéma d’achats de soins.
  3. openIMIS : faisabilité et estimation des coûts. Le but est de personnaliser openIMIS afin de satisfaire les besoins de gestion des soins du paludisme.
  4. Mise en œuvre du pilote openIMIS afin d’enregistrer et suivre les prestations sanitaires du paludisme servies en vue de disposer d’informations sur leurs fréquences et leurs coûts et ainsi disposer d’un outil de suivi des financements dédiés à cette maladie.

Swiss TPH s’est entouré de compétences expertes pour la réalisation de ce mandat avec d’une part Oeconomia-Expertise pour les question d’économie de la santé et Y-Note pour les aspects informatiques au Cameroun.

openIMIS est un outil open source qui est actuellement utilisée avec succès dans la campagne de gratuité des soins et traitements VIH au Cameroun ainsi qu’au sein réseau d’assurance santé communautaire BEPHA et dont un pilote est en préparation pour le Chèque Santé, toujours au Cameroun. Le projet s’orientera donc selon les bonnes pratiques observées notamment au Cameroun. En outre le projet sera piloté en étroite collaboration avec l’équipe du Programme National de lutte contre le paludisme (PNLP) et celle de BACKUP Santé à la GIZ.

Les quatre groupes d’activités suivants seront mis en œuvre successivement :

  1. Estimation des coûts des services et soins liés au traitement du paludisme :
  2. Conception du pilote et de la feuille de route :
  3. openIMIS : faisabilité et estimation des coûts.
  4. Mise en œuvre du pilote openIMIS.

En fin de projet il est attendu que le pilote openIMIS soit fonctionnel et que les possibilités de sa mise à l’échelle soient connues.

Le projet portera une attention particulière à la cible spécifique des enfants de moins de 5 ans, bénéficiaire de la politique nationale de gratuité des soins du paludisme ainsi que des femmes enceintes. Il s’agira donc dans les différentes composantes du projet (étude de coût, feuille de route, suivi des données avec openIMIS) de distinguer spécifiquement ces cibles du reste de la population.

On relève les partenaires directs de mise en œuvre suivants :

  • Les partenaires de pilotage, notamment GIZ BACKUP/P4H, le Programme National de Lutte contre le Paludisme (PNLP) du Ministère de la santé (MINSANTE) au niveau central.
  • Les partenaires de mise en œuvre identifiés à ce stade : la cellule informatique du MINSANTE ; les autorités sanitaires régionales, FRPS et formations sanitaires (FOSA) impliquées dans les activités notamment pour l’étude de coûts et pour le pilote openIMIS.
